GÜVENLİ CERRAHİ KONTROL LİSTESİNİN ETKİNLİĞİ: SİSTEMATİK İNCELEME

Abstract

       Amaç: Güvenli Cerrahi Kontrol Listesinin
etkinliğinin değerlendirildiği araştırmaların gözden geçirilmesi ve gelecekteki
araştırmalara ışık tutması amacıyla gerçekleştirilmiştir.

    
Yöntem:
Tanımlayıcı
çalışmada Pubmed/MEDLINE ve Cochrane
Library veri tabanları üzerinden “güvenli cerrahi kontrol listesi”, “dünya
sağlık örgütü güvenli cerrahi kontrol listesi”,“etkinlik” ve “etki” anahtar
kelimeleri kullanıldı. Kasım 2015 öncesindeki yayınlanmış 49 makale ele alındı.
Etkinlik sonuçları uyum, komplikasyon ve mortalite oranı, hasta güvenliği, ekip
iletişimi, çalışan ve hasta memnuniyeti ve maliyet açısından değerlendirildi.

    
Bulgular:
  Uyumun en kolay olduğu bölüm insizyon
öncesiyken, en zor olduğu bölüm ise çıkış kontrolü evresidir. Listenin
kullanımı sonrası komplikasyon ve mortalite oranlarında anlamlı bir düşüş
bulunmaktadır. Listenin kullanımı plansız ameliyathaneye dönüşlerde, yeniden
ameliyat olma oranında ve istenmeyen etkilerde azalma olurken hasta kimliğinin
doğrulanmasında ve hastanın tıbbi durumunda farkındalıkta artma görüldü. Ekip
iletişiminde artma ve iletişim hatalarında düşme görüldü. Çalışanlar listenin
uygulanmasını %50 ile %95 oranında olumlu buldu. Ameliyathane harcamaları
düştü. Hasta memnuniyeti %57.1’den %90.8’e yükseldi.







    
Sonuçlar:
Listenin
kullanımı ile komplikasyon ve mortalite oranlarındaki ciddi azalma olduğu,
ameliyathanede ekip çalışmasını ve iletişimi geliştirdiği, ameliyathaneden
plansız geri dönüşleri azaltması, ameliyat olma oranını azaltması, hasta
kimliğinin doğrulanma sıklığını arttırması ve cerrahinin istenmeyen etkilerini
azaltması nedeniyle hasta güvenliğini geliştirmektedir. GCKL ekip üyeleri
arasında iletişimi, işbirliğini ve hasta güvenliğini geliştiren bir araç olarak
önerilmektedir.
