Alzheimer Hastalığı Kalça Kırığında Beş Yıllık Mortaliteyi Etkileyen Önemli Bir Risk Faktörü Müdür?

Abstract
Amaç: Erişkin hastalarda kalça kırıklarında uygulanan parsiyel kalça artroplastisinin 5 yıllık mortalitesi belirlenmeye çalışılmıştır. Mortaliteyi etkileyebilecek risk faktörleri analiz edilip Alzheimer hastalığının risk faktörleri arasında yer alıp almadığının belirlenmesi amaçlanmıştır.
Gereç ve Yöntemler: Çalışmaya 2009-2012 yıllarında kalça kırığı tanısıyla başvuran 145 hastadan kriterlere uygun parsiyel kalça artroplastisi yapılan 50 hasta dahil edildi. 5 yıllık mortaliteyi etkileyebileceği öngörülen risk faktörleri (yaş, cinsiyet, taraf, ASA (American Society of Anesthesiologists) oranı), kullanılan Eritrosit Süspansiyonu (ES) miktarı, uygulanan anestezi türü, yoğun bakım ihtiyacı, ameliyat süresi, Alzheimer hastalığı tanısı alıp almaması) analiz edildi.
Bulgular: Hastalardan 5 yıl içinde exitus olanlarla 5 yıl sonunda yaşayanlar karşılaştırıldığında yaş, cinsiyet, taraf, ASA oranı, kullanılan ES miktarı ve uygulanan anestezi türü açısından anlamlı bir fark gözlenmedi. Hastanede kalış süresi, yoğun bakım ihtiyacına göre karşılaştırıldığında ise istatistiksel olarak anlamlı bir farklılık olduğu görüldü (p<0,05). Ameliyat süresi 95 dakika ve üstü olgularda 5 yıllık mortalitenin daha yüksek olduğu gözlemlendi (p<0,05). 5 yıl sonunda mortalitenin %68 olduğu gözlemlendi. Ancak Alzheimer hastalığı tanısı olan hastalarda 5 yıllık mortalitenin %100 olduğu görüldü (p<0.03).
Sonuç: Çalışmamızda yoğun bakım ihtiyacı ve ameliyat süresi 95 dakika ve uzun olanların 5 yıllık mortalitesinin yüksek olduğu görülmüştür. Alzheimer hastalığının 5 yıllık mortaliteyi etkileyen önemli bir risk faktörü olduğu bulunmuştur.
