Tek Sağlık Uygulamalarının Değerlendirilmesi: Standardize Edilmiş Güncel Yöntemler

Abstract

Tek Sağlık yaklaşımı, hayvan-insan-ekosistem etkileşiminde halk sağlığı sorunlarının ele alınmasında kritik bir araç olarak önem kazanmaya devam etmektedir. Küresel düzeyde bu kavrama geniş bir destek vardır ve bu durum dünya çapında çeşitli Tek Sağlık girişimlerinin kurulmasına yol açmıştır. Tek Sağlık topluluğu için güncel bir zorluk, Tek Sağlık'ın sürdürülebilirliğini sağlamak için eylem çağrısının ve temel prensiplerin ilerisine nasıl geçileceğidir. Gerçek dünyaya ilişkin verilerin yetersizliği, insan-hayvan-çevre sağlığı eksenindeki eksikliklerin tespit edilmesini engellemekte, bu durum politika ve uygulamaların şekillendirilmesinde Tek Sağlık yaklaşımının uygulanmasını zorlaştırmakta; önleme ve kontrol faaliyetlerinin gecikmesine veya eksik kalmasına yol açmaktadır. Bu nedenle Tek Sağlık'ın hastalıkların önlenmesi ve kontrolü, antimikrobiyal direnç, gıda güvenliği ve çevre sağlığı alanlarındaki ek faydalarını gösteren standardize edilmiş analizler önemlidir ve bunların değerlendirilmesi gerekmektedir. Tek Sağlık Değerlendirme Ağı (NEOH), Küresel Sağlık Güvenliği (GHS) Endeksi, Çevresel Performans Endeksi (EPI) ve Küresel Tek Sağlık Endeksi (GOHI) gibi uygun bir değerlendirme şeması ile birlikte iyi yapılandırılmış kavramsal sistemler, mevcut durumun daha iyi anlaşılmasına ve Tek Sağlık uygulaması için belirli sosyo-ekolojik ortamlara kolayca uyarlanmış hedef ve stratejilerin belirlenmesine yardımcı olabilmektedir. Tek Sağlık uygulamalarının değerlendirilmesi için standardize edilmiş yöntemlerin geliştirilmesi, Tek Sağlık girişimlerinde daha uzun vadeli yeni kazanımları daha kalıcı hale getirmek için büyük bir fırsat sunmaktadır.

Evaluation of One Health Implementations: Current Standardized Frameworks

Abstract

The One Health approach continues to gain recognition as a critical tool to address public health issues at the animal–human-ecosystem interface. At a global level, there is broad support for the concept, which has led to the establishment of several One Health initiatives around the world. A current challenge for the One Health community is how to go beyond the call for action and key principles to ensure the sustainability of One Health. The dearth of real-world evidence has hindered the identification of gaps in the human–animal–environment health nexus, which hampers the application of a One Health approach in shaping policies and practice, and results in delayed or incomplete prevention and control measures. Therefore, formal standardized analyses showing the added benefits of One Health in disease prevention and control, antimicrobial resistance, food safety, and environmental health is important and need to be assessed. A well-constructed conceptual systems, along with an appropriate evaluation scheme, such as Network for Evaluation of One Health (NEOH), Global Health Security (GHS) Index, The Environmental Performance Index (EPI), and Global One Health Index (GOHI) can help to better understand the current situation and to set goals and strategies for One Health implementation, readily tailored to specific social-ecological settings. The development of a potential evaluation tool for One Health performance framework provides a great opportunity to make new capacities more durable for longer-term gains in One Health initiatives.
