Nadir Bir Menenjit ve Beyin Absesi Etkeni Olarak Streptococcus constellatus

Abstract

Menenjit; acil müdahale gerektiren durum olup, beyin ve spinal kordu çevreleyen pia ve araknoid zarın infl amasyonu
iken beyin apseleri ise beyinin parankim dokusunda gelişen süpüratif enfeksiyonlardır. Beyin abseleri; travma
ve cerrahi müdahale gibi dış odaklı olabileceği gibi konjenital kalp hastalığı, endokardit, intratorasik ve abdominal
enfeksiyon temelli sepsis, diş çürüğü, otitis media veya sinüzite bağlı bir komplikasyon olarak da ortaya çıkabilir. Bu
olgu sunumunda, yüksek ateş, kusma ve omuz ağrısı olan, larenks kanseri tanısı ile trakeostomi öyküsü bulunan 59
yaşında bir erkek hastayı sunuyoruz. Hastadan menenjit ön tanısı ile yapılan periferik kan kültürü ve beyin omurilik
sıvısı kültürü sonucunda Streptococcus constellatus izole edilmiştir. Menenjit için antibiyotik tedavisi başlanan hastada
tedaviden beklenen cevap alınamayınca yapılan bilgisayarlı beyin tomografi si sonucunda yaklaşık 10cm çapında beyin
absesi görüntülenmiştir. Hastada daha önceden mevcut olan trakeostomi beyin absesi için predispozan faktör olarak
değerlendirilmiştir. Uygulanan antibiyotik tedavisi sonucu abse gerilemiş ve 5cm’ye kadar küçülmüştür. Buna rağmen
hasta yatışının 60. gününde kaybedilmiştir. Predispozan faktörlerin varlığında, vücutta normal fl ora elemanı olarak
bulunan birçok mikroorganizmanın ciddi enfeksiyonlara yol açabileceği unutulmamalıdır.

A Case of Meningitis and Brain Abscess Due to Streptococcus constellatus

Abstract

Meningitis is the infl ammation of the pia and arachnoid membranes surrounding the brain and spinal cord and requires
urgent medical intervention, whereas the brain abscess is the suppurative infection occurs in the parenchymal tissue
of the brain. Brain abscesses may result from trauma and surgical intervention, as well as may originate as a complication
of congenital heart disease, endocarditis, intrathoracic and abdominal infection-based sepsis, tooth decay, otitis
media or sinusitis. In this case report, we present a 59-year-old man with a history of tracheostomy with a diagnosis of
laryngeal carcinoma with high fever, vomiting and shoulder pain. Peripheral blood culture samples cerebrospinal fl uid
were drawn with the prediagnosis of meningitis and the samples yielded Streptococcus constellatus. The patient received
antibiotic therapy for meningitis; but the expected response to the treatment was not achieved. The brain abscess
of about 10 cm in diameter was visualized as the result of computed tomography. The pre-existing tracheostomy in
the patient was considered as a predisposing factor for brain abscess. After he antibiotic treatment, the diameter of
the abscess reduced to 5 cm. However, the patient died on the 60th day of admission. It should be noted that many
microorganisms of the normal human fl ora might cause serious infections in the presence of the predisposing factors.
