Dişeti Gülümsemesi ve Papil Kaybına Sahip Bir Olgunun Tedavisinde Multidisipliner Yaklaşım: Olgu Sunumu

Abstract
Dişetinin fazla göründüğü dişeti gülümsemesi estetik algıyı olumsuz yönde etkileyen multi-faktöriyel bir durumdur. Bu olgu sunumunda dişeti gülümsemesi ve papil kaybı olan bir hastanın multidisipliner bir yaklaşımla tedavisi sunulmuştur. Otuz yaşında kadın hasta gülme sırasında diş etlerinin fazla görünmesi ve üst ön dişleri arasındaki diş eti görünümünden rahatsız olduğunu belirterek kliniğimize başvurdu. Hastadan alınan teşhis ölçüleri, fotoğraflar ve kapanış kayıtları sonrası wax-up model ve dijital gülüş tasarımı ışığında bir periodontal rehber plak hazırlandı ve 15-25 numaralı dişler arasındaki bölgeye gingivektomi ve gingivoplasti işlemleri uygulanarak kuron boyu uzatma işlemi yapıldı. 11-21 numaralı dişler arasındaki papil kaybının düzeltilmesinde ise palatinal bölgeden alınan serbest diş eti grefti kullanıldı. Üç haftalık iyileşme sürecinin ardından, 15-25 numaralı dişler arası bölge, feldspatik seramik veneer restorasyonlarla rehabilite edildi. Multidisipliner yaklaşım, karmaşık tedavi olgularının görsel, fiziksel modelleme teknikleri kullanılarak planlanması ve rehberlikli olarak yürütülmesi klinik başarı ve öngörülebilir sonuçlar elde edebilmek için son derece önemlidir.

A Multidisciplinary Treatment Approach Of A Case With Gummy Smile And Lost Papilla: Case Report

Abstract
The gummy smile is a multifactorial condition that the gingiva appears too much and negatively affects the aesthetic appearance. The aim of this case report was to present a multidisciplinary treatment approach of a patient with gummy smile and loss of papilla.
A 30-year-old female patient admitted to our clinic with the complaint of the excessive gingival display during laughing and the appearance of gingiva between her upper central incisors. After achieving the diagnostic impressions, photographs, and occlusal records, a periodontal guide plate was fabricated according to the wax-up model and digital smile design. Then the plaque-guided gingivectomy and gingivoplasty surgical procedures were performed between 15-25 numbered teeth. A free gingival graft was taken from the palatinal area and used to correct the loss of papilla between 11-21 numbered teeth. The following three-week healing period, the 15-25 numbered teeth were rehabilitated by feldspathic ceramic veneer restorations.
The planning and practice of the multidisciplinary and complex treatment using visual and actual modeling techniques are very important for clinical success rates and predictable results.
