Yaşlı Bireylerde Malnutrisyon Ve Ortostatik Hipotansiyon Arasında İlişki Var Mı?

Abstract
Amaç:Malnutrisyon ve ortostatik hipotansiyon(OH) yaşlı popülasyonda sık görülen iki geriatrik sendromdur. Bu iki geriatrik sendrom yaşlılarda önemli morbidite ve mortalite nedeni olarak karşımıza çıkmaktadır. Bu çalışmanın amacı malnutrisyon ve OH arasında ilişki olup olmadığını tespit etmektir. Materyal-Metod: 2013-2016 yılları arasında 599 yaşlı bireyi inceleyen retrospektif ve kesitsel bir çalışma olarak düzenlendi. Bütün hastaların demografik verileri,son bir yılda düşme hikayesi dahil komorbiditeleri, vücut kitle indeksi (VKİ) değeri, ayrıntılı geriatrik değerlendirme parametreleri ve laboratuar bulguları ele alındı. OH, supin pozisyondan ayağa kalkar pozisyona geçildiğinde sistolik ve/veya diastolik kan basınçlarında sırasıyla 20 ve/veya 10 mmHg düşüş olması olarak tanımlandı. Bulgular: Katılımcıların yaş ortalaması 74+8.09 olup, çoğunluğu kadınlardan (%63.2) oluşmaktadır. Malnutrisyon-risk olanların oranı %42.5 idi. Düşme, depresyon ve hipertansiyon malnutrisyon-risk grubunda daha fazla izlendi (p<0.05). Mini-Mental Durum Değerlendirme (MMSE), Montreal Kognitif Değerlendirme (MOCA) ve Kognitif Durum Testi (COST), Günlük Yaşam Aktivite(GYA) parametreleri ve denge-yürüme skorları malnutrisyon-risk grubunda düşük izlendi (p<0.05). Malnutrisyon-risk grubunda OH oranı daha fazla idi (p=0.025). Sonuç: Malnutrisyon ve OH arasında güçlü bir ilişki olabilir ve bu bulgu ileri çalışmalarla desteklenmelidir.
