Yaşlılarda Depresyon Düşme Sıklığı ile İlişkilendirilebilir mi?

Abstract

Amaç: Düşme, yaşlı kişilerde yaygın olarak görülen ve bireylerin bağımsızlığını tehdit eden önemli bir sağlık sorunudur. Düşme için önlenebilir risk faktörlerinin tanınması ve ortadan kaldırılması bireyin yaşam kalitesi için önemlidir. Yaşlı bireylerde sık görülen sendromlardan biri olan geriatrik depresyonun tanınması ve tedavisinin başlanması ayrıntılı geriatrik değerlendirme ile mümkün olmaktadır. Bu çalışmanın amacı, yaşlılarda sık görülen iki sendrom olan geriatrik depresyon ile düşme arasındaki ilişkiyi ortaya koymaktır.

Gereç ve Yöntemler: Çalışmaya 60 yaş ve üzeri 1372 hasta alındı. Çalışmamız kesitsel retrospektif olarak dizayn edildi. Katılımcıların sosyodemografik verileri, son bir yıldaki düşme sıklığı öyküsü, geriatrik depresyon ölçeği puanları, yürüme ve denge ölçeklerini içeren Ayrıntılı Geriatrik Değerlendirme verileri hasta takip dosyalarından incelenerek değerlendirildi. Elde edilen sonuçlarla prevelans analizi yapıldı.

Bulgular:
Düşme, depresyon tanısı olan grupta, depresyon tanısı olmayan gruptan anlamlı olarak yüksekti (sırasıyla %35,2; %26,7, p=0,003). Her iki grup yaş açısından benzerlik göstermekteydi. Depresyon tanısı alanlarda kadın cinsiyet oranı daha fazlaydı (p<0,001). Düşme, depresyon tanılı yaşlı olgularda ‘’Zamanlı Kalk ve Yürü Testi’’ ve Performans Odaklı Mobilite Değerlendirmesi (POMA) skorları ile korele bulundu (p<0,001).

Sonuç: Depresyon ve düşme sıklığı arasında anlamlı bir ilişki bulunmaktadır. Düşme öyküsü olan depresyon tanılı hastaların yürüme ve denge durum değerlendirmesi ve bu durumların yönetimi gözden geçirilmelidir. Depresyonu olan yaşlı bireyler düşme açısından titizlikle sorgulanmalı ve takip edilmelidir. Bu konuda yapılacak destekleyici çalışmalara ihtiyaç vardır.

May Depression Be Associated with the Frequency of Falls in the Elderly?

Abstract

Aim: Falls are an important health problem that is common in older people and threatens individual’s indipendence. Recognition and elimination of preventable risk factors for falls is important for individual’s quality of life. Recognizing and initiating the treatment of geriatric depression, which is one of the most common syndromes in elderly individuals, is possible with Comprehensive Geriatric Assessment. The aim of this study is to reveal the relationship between two common syndromes in elderly, geriatric depression and falls.

Material and Methods: 1372 patients aged 60 and over were included in the study. Our study was designed as a cross-sectional retrospective. Sociodemographic data of the participants, history of falls frequency in the last year, geriatric depression scale scores, Comprehensive Geriatric Assessment data including gait and balance scales were analyzed from patient follow-up files. Prevalence analysis was performed with the results obtained.

Results: Falls was significantly higher in the group with a diagnosis of depression than the group without a diagnosis of depression (35.2%; 26.7%,respectively, p=0.003). There was no significant age difference between the two groups. The female gender ratio was higher in those diagnosed with depression. Falls was found to be correlated with ‘’Timed Up and Go Test’’ and Performance-Oriented Mobility Assessment (POMA) scores in elderly patients diagnosed with depression (p<0.001).

Conclusions: There is a significant relationship between depression and frequency of fall. The gait and balance status assessment of patients diagnosed with depression with a history of falls and the management of these conditions should be reviewed. Elderly individuals with depression should be meticulously questioned and followed up in terms of falls. Supportive studies are needed in this regard.
