Postpartum Depresyon ve Maternal D Vitamini Düzeyleri Arasındaki İlişki Hakkında Bir Sistematik Derleme

Abstract

Postpartum Depresyon (PPD) kadınların %10-15’ini etkileyen ve hem anne hem de yeni doğanın sağlığında olumsuzluklara neden olan, doğumdan sonraki ilk dört hafta içinde ortaya çıkan yaygın bir duygu durum bozukluğudur. Son yıllarda yapılan çalışmalarda gebelikte sıklıkla görülen D vitamini yetersizliğinin postpartum depresyon ile olan ilişkisi üzerinde durulmaya başlanmıştır. Bu sistematik derleme çalışmasında, maternal D vitamini düzeylerinin postpartum depresyon üzerine olan etkilerinin araştırıldığı çalışmalar gözden geçirilerek, konuya dikkat çekmek amaçlanmıştır.

Araştırmaya dâhil edilen 6 makalede; gebeliğin çeşitli aylarında alınan kan örneklerindeki serum D vitamini düzeyleri ile gebelik sonrasında uygulanan Edinburgh Doğum Sonrası Depresyon Ölçeği skorları arasındaki ilişki incelenmiştir. Yürütülen çalışmaların %66,66’sında prenatal 25(OH)D3 seviyeleri ile yüksek EDPS skorları arasında anlamlı bir ilişki saptanırken, serum D vitamini düzeyi çok düşük olanlarda PPD riskinde daha fazla artış olduğu bulunmuştur. Çalışmaların %33,34’ünde ise gebelikteki 25(OH)D3 konsantrasyonları ile PPD arasında hiçbir ilişki olmadığı; bu çalışmalardan birinde de en yüksek D vitamini konsantrasyonuna sahip kadınlar arasında PPD ‘nin riskinin artmış olduğu saptanmıştır.

Gebelikte D vitamini yetersizliği ve postpartum depresyon sıklıkla rastlanan bir durum olduğu için gebelikte ve sonrasında mutlaka D vitamini takviyesi yapılması gerekmektedir.
