An Evaluation of the Relationship Between Roots of Maxillary Anterior Teeth and Neighboring Anatomical Structures in Children: A Cone Beam Computed Tomography Study

Abstract

ABSTRACT
Background: In our study, we aimed to evaluate the distances between the roots of maxillary incisors and the nasopalatine canal and the floor of the nasal cavity, and the buccal cortical bone thickness at the apices of the roots of these teeth by Cone Beam Computed Tomography (CBCT) in children in the permanent dentition period.
Materials and Methods: CBCT images of 49 patients aged 6-14 years were evaluated. In the sagittal plane, the distances of the apices of the maxillary central teeth with the nasopalatine canal and with the floor of the nasal cavity were evaluated. Buccal cortical bone thickness at the apex of the roots of maxillary anterior teeth was examined. These data were compared in terms of gender and whether the teeth had open or closed apices.
Results: When the mean distance of the maxillary central teeth (11,21) to the nasopalatine canal was evaluated in terms of the open/closed apex status of the teeth and gender, there was no significant relationship (p>0.05). Among the maxillary anterior teeth (11, 21,12, 22), the root apex of tooth 22 was the farthest from the floor of the nasal cavity and the root apex of tooth 12 was the closest. It was found that the mean buccal cortical bone thickness of maxillary anterior teeth 11 and 21 with open apex was significantly higher than those with closed apex (p<0.05).
Conclusions: The relationship of the maxillary anterior teeth with the anatomical structures in this region is important due to the frequent occurrence of traumatic dental injuries such as intrusion or lateral luxation, overflow filling during faulty root canal treatment, and apical surgery or implantation in this region.

Çocuklarda Maksiller Anterior Dişlerin Kökleri ile Komşuluğundaki Anatomik Yapılar Arasındaki İlişkinin Değerlendirilmesi: Konik Işınlı Bilgisayarlı Tomografi Çalışması

Abstract

ÖZ
Amaç: Çalışmamızda kalıcı dişlenme dönemindeki çocuklarda maksiller keser dişlerin kökleriyle nazopalatin kanal, nazal kavite tabanı arasındaki mesafeyi ve bu dişlerin kök apekslerindeki bukkal kortikal kemik kalınlığını Konik Işınlı Bilgisayarlı Tomografiyle (KIBT) değerlendirmek amaçlanmıştır.
Gereç ve Yöntemler: 6-14 yaş aralığındaki 49 hastanın KIBT görüntüleri değerlendirilmiştir. KIBT’de sagital düzlemde maksiller santral dişlerin apeksleriyle nazopalatin kanal, maksiller anterior dişlerin apeksleriyle ise nazal kavite tabanı arasındaki mesafe değerlendirilmiştir. Maksiller anterior dişlerin köklerinin apeksindeki bukkal kortikal kemik kalınlığı incelenmiştir. Bu veriler cinsiyet ve dişlerin açık/kapalı apeksli olma durumu açısından karşılaştırılmıştır.
Bulgular: Maksiller santral dişlerin (11,21) nazopalatin kanala olan ortalama uzaklığı, dişlerin açık/kapalı apeksli olma durumu ve cinsiyet açısından değerlendirildiğinde aralarında anlamlı bir ilişki bulunamamıştır (p>0.05). Nazal kavite tabanına maksiller anterior dişlerden (11,21,12,22) 22 nolu dişin kök apeksinin en uzak, 12 nolu dişin kök apeksinin ise en yakın olduğu görülmüştür. Maksiller anterior dişlerden 11 ve 21 nolu dişlerin açık apeksli olanlarının kapalı apekslilere göre ortalama bukkal kortikal kemik kalınlığının anlamlı olarak daha fazla olduğu bulunmuştur (p<0.05).
Sonuçlar: Maksiller anterior dişlerde, intrüzyon veya lateral lüksasyon gibi travmatik dental yaralanmaların sık görülmesi, hatalı kök kanal tedavisi esnasında taşkın dolgu görülebilmesi, bu bölgeye apikal cerrahi veya implant uygulanması gibi nedenlerden dolayı bu bölgedeki anatomik yapılarla olan ilişkisi önemlidir.
