Tip 2 Diyabetik Hastalara Verilen Diyabet Eğitiminin İçeriği ve Hastaların Hastalık Tutumu

• Amaç:
  Bu çalışmada, Türkiye’nin Güneydoğu bölgesinde dahiliye klinik hemşireleri tarafından verilen diyabet eğitiminin içeriği ve bu hemşirelerin bakım verdiği diyabet hastalarının hastalık tutumunu belirlemek amaçlanmıştır. Gereç ve Yöntem: Tanımlayıcı desende planlanan araştırmanın yürütülmesi için Gaziantep Üniversitesi Etik Kurul onayı alınmıştır. Araştırmaya, Ocak-Mart 2015 tarihleri arasında Güneydoğu bölgesinde bulunan beş kurumun dahiliye kliniklerinde “Tip 2 Diyabet” tanısı almış hastalar ile bu hastalara bakım veren hemşireler gönüllülük esasına dayanarak dahil edilmiştir. Araştırmada hasta ve hemşireler için ayrı formlar kullanılmıştır. Hastalar için Türkçe geçerlik ve güvenirlik çalışması Özcan (1999) tarafından yapılan “Diyabet Tutum Ölçeği” (DTÖ) ve “Hasta Tanıtıcı Form”, hemşireler için “Diyabet Eğitimi Değerlendirme Formu” kullanılmıştır. Verilerin analizinde tanımlayıcı istatistikler yapılmıştır.Bulgular: Çalışmaya alınan hemşirelerin %70.8’inin kliniklerinde baktığı Tip 2 diyabet hastalarına diyabet eğitimi verdiği ve verilen eğitimin %85.1 oranında bireysel eğitim olduğu belirlenmiştir. Aynı kliniklerde bakım alan hastaların hastalık tutumu değerlendirildiğinde; hastaların %95.5’i hastalığı ile ilgili negatif tutuma sahiptir ve DTÖ toplam puanları 2.07±0.42’dir. Tüm alt boyutlardan alınan puan ortalamasına bakıldığında, en düşük tutumun 0.85±1.13 ile “Hasta Otonomisine Karşı Tutum”, en yüksek tutumun 3.06 ±1.00 ile “İnsüline Bağımlı Olmayan Diyabetin Ciddiyeti” olduğu belirlenmiştir. En düşük düzeyde çıkan “Hasta Otonomisine Karşı Tutum” alt boyutu ile diyabet tedavisi ve kontrolü için diyabetlilerin bireysel yönetime bakışı, rol ve sorumluluklarını algılama ve kabul etme düzeyleri değerlendirilmektedir. Bu alt boyutta ortalamanın düşük olması nedeniyle hastaların bireysel hastalık yönetimi konusunda başarısız olduğu belirlenmiştir. Sonuç: Bu çalışma sonuçlarına göre Güneydoğu’da diyabet hastalarına hemşire tarafından verilen eğitimin etkili olmadığı ve hastaların da diyabetin bireysel yönetiminde iyi olmadığı söylenebilir. Bu nedenle klinik hemşirelerinin diyabet eğitimi gözden geçirilmeli, hastaların hastalık tutumu arttırılmalıdır.

The Content of Diabetes Education in Type 2 Diabetic Patients and Their Attitude Towards the Disease

Objective: The aim of this study is to analyse the diabetes education content offered by internal clinical nurses in the Southeast part of Turkey and to determine the attitudes of the patients receiving care from these nurses towards the disease. Materials and Method: In order to conduct

this descriptive researchethical approval was received from the Ethics Committee of Gaziantep University. The research included patients diagnosed with “Type 2 Diabetes” and the caregiver nurses on a voluntary basis at the internal clinics of five institutions located in the Southeastern
Region of Turkey between the dates of January-March 2015. In the research, separate forms were used for the patients and nurses. The patients were applied “Patient Identification Form” and the Turkish version of the “Diabetes Attitude Scale” (DAS) of which validity and reliability was studied by Özcan (1999) while the nurses were applied “Diabetes Education Evaluation Form”. For the analysis of data, descriptive analysis was performed. Findings: It was determined that 70.8% of the nurses offered diabetes education to patients with Type 2 diabetes under their care in clinics and that 85.1% of the education was individual education. When the attitude of the patients receiving nursing care in the same clinics was evaluated, it was seen that 95.5% had a negative attitude towards their disease and that their total DAS score was 2.07±0.42. When the mean score received from all subdimensions is considered, it was determined that the lowest attitude was on the sub-dimension of “Attitude Towards Patient Autonomy” with a value of 0.85±1.13 while the highest attitude was on the sub-dimension of “The severity of Non-insulin Dependent Diabetes Mellitus” with a value of 3.06 ±1.00. With the “Attitude Towards Patient Autonomy” which received the lowest value, the attitude of patients with diabetes towards individual management and the levels of their perception and acceptance of roles and responsibilities regarding the treatment and control of diabetes are evaluated. It was determined that patients were unsuccessful about individual disease management due to this sub dimension’s low mean score. Conclusion: In conclusion, it could be said that education offered to patients with diabetes by the nurses in Southeast Region of Turkey was not effective and that patients were not adequate regarding the individual management of diabetes. Therefore, the diabetes education of clinical nurses should be revised and the attitude of patients towards the disease should be improved.
