Fertiliteyi Geliştirmede Prekonsepsiyonel Bakım: İnfertil Çiftler İçin Örnek Bir Holistik Bakım Şeması

Abstract
İnfertilite, çiftlerin bir yıl veya daha fazla süreyle düzenli ve korunmasız cinsel ilişkide bulunmasına rağmen klinik gebeliğin sağlanamadığı durumdur. Dünya Sağlık Örgütü’ne göre dünya genelinde birçok çift infertilite sorunu ile karşı karşıyadır. İnfertilite, genetik ve epigenetik nedenlerden kaynaklanan karmaşık etiyolojiye sahip bir durumdur. Çiftlerin prekosepsiyonel dönemde maruz kaldığı fiziksel ve psikolojik etkenler nedeniyle oluşan epigenetik değişiklikler, gamet hücrelerinde DNA hasarının artmasına yol açmaktadır. Bu durumda, konsepsiyon olasılığı azalmakta ve ayrıca erken gebelik kayıpları artmaktadır. Bu açıdan, infertil çiftlere prekonsepsiyonel bakım sağlanması daha iyi gebelik sonuçları elde edilmesini sağlayabilir. Gebelik öncesi sağlığın geliştirilmesini hedefleyen prekonsepsiyonel bakım, sağlıklı gebelikler ve yenidoğanlar için uygulanan girişimleri içermektedir. Yapılan çalışmalar ile, prekonsepsiyonel bakımın fertiliteyi geliştirmede etkili olduğu gösterilmiştir. Bu nedenle, infertil çiftlerin bakımında mutlaka prekonsepsiyonel bakım yer almalı ve holistik (bütüncül) bir yaklaşım ile sunulmalıdır. Holistik hemşirelik bakımı, bireyi sadece fiziksel değil tüm yönleriyle ele alan bir yaklaşım olduğundan infertil çiftler için önem arz etmektedir. Yapılan çalışmalar çiftlerin de bireye özgü bütüncül bakımı tercih ettiğini göstermektedir. Çiftler için oldukça önemli olan bu bakımda, bireylerle en sık iletişim kuran sağlık profesyonellerinden olan hemşirelerin aktif rol alması önemlidir. Bu derlemede infertil çiftlerde prekonsepsiyonel bakımın önemine dikkat çekmek, sağlık profesyonellerine yönelik holistik bakım girişimleri sunmak ve hemşirenin rollerini ortaya koymak hedeflenmiştir.

Abstract
Infertility is a condition in which couples have had regular and unprotected sexual intercourse for a year or more but are unable to conceive. According to World Health Organization, it has been stated that many couples around the world are experiencing infertility issues. Infertility is a condition with a complicated etiology caused by genetic and epigenetic factors. Increased DNA damage in gamete cells is caused by epigenetic changes caused by physical and psychological factors that couples are exposed to during the preconception period. As a result, the likelihood of conception decreases and the risk of miscarriage rises. Infertile couples who receive preconception care have a better chance of becoming pregnant. Preconception care, which aims to improve pre-pregnancy health, includes preconception interventions that promote healthy pregnancies and new-borns. Studies have shown that preconception care is effective in improving fertility. Therefore, preconception care should be included in the care of infertile couples and should be presented with a holistic approach. For infertile couples, holistic nursing care is essential because it addresses all aspects of an individual, not just the physical. Couples also prefer individualized holistic care, according to studies. It is important that nurses who interact with patients on a regular basis play an active role in their care. In this review, it is aimed to highlight the importance of preconception care in infertile couples, to present holistic care practices and to reveal nurse’s role.
