PANKREATİK ADENOKANSERDE SİRKÜLASYONDAKİ PROİNFLAMATUAR SİTOKİN DÜZEYLERİNİN KLİNİK ÖNEMİ

Abstract

Amaç: Tümör nekroz faktör alfa (TNF-α) ve interlökin 1 beta (IL-1β) pankreatik adenokanser (PA) oluşumunda anahtar rol oynamaktadır. Bu çalışmada PA’lı hastalarda bu moleküllerin serum düzeylerinin klinik önemini belirlemeyi amaçladık.

Gereç ve yöntem: Çalışmamıza patolojik olarak konfirme edilen PA tanılı 33 hasta ile yaş-cinsiyet olarak eşleştirilmiş 30 sağlıklı kontrol alındı. Serum proinflamatuar sitokin düzeyleri, ELISA yöntemiyle değerlendirildi.

Bulgular: Ortanca tanı yaşı 59 (aralık, 32-84) olarak saptandı. Cerrahi girişim uygulanan %27 hastanın; %15’ine pankreatikoduodenektomi operasyonu ve %12’sine palyatif cerrahi uygulandı. Serum proinflamatuar sitokin düzeyleri, hasta grubunda kontrol grubuna göre anlamlı olarak yüksek bulundu (her iki belirteç için; p<0,001). Ayrıca, sarılığı, büyük tümörü, metastatik hastalığı veya karaciğer metastazı olan hastalarda IL-1β düzeyi yüksek olarak tespit edildi (sırasıyla, p=0,008, p=0,002, p=0,008 ve p=0,03). Ancak, PA’lı hastalarda serum proinflamatuar sitokin düzeyleri ile prognoz arasında anlamlı bir ilişki saptanmadı.

Sonuçlar: Serum proinflamatuar sitokin düzeylerinin PA’lı hastalarda tanısal değeri bulunmaktadır ancak prognostik önemi saptanamamıştır.

Abstract

Objective: Tumor necrosis factor-α (TNF-α) and interleukin 1β (IL-1β) play key roles in pathogenesis
of patients with pancreatic adenocarcinoma (PA). The objective of this study
was to determine the clinical significance of serum levels of these molecules
in PA patients.



Materials and Methods: A total of 33 patients with a pathologically confirmed diagnosis of PA
and age- and sex-matched 30 healthy controls were enrolled in this study. Serum
proinflammatory cytokine concentrations were determined by the ELISA method.



Results: The median age at diagnosis was 59 years,
range 32 to 84 years. Of the
patients who
had undergone surgical intervention (27%), 15% had had pancreaticoduodenectomy
and palliative surgery was performed in the remaining 12%.
The baseline serum proinflammatory cytokine levels were significantly higher in patients with PA than in
the healthy control group (both for TNF-α and IL-1β; p<0.001). Additionally,
serum IL-1β levels of the patients with
jaundice, large tumor size,
metastasis or liver metastasis were detected to be significantly higher
compared with others
(p=0.008,
p=0.002, p=0.008, and p=0.03, respectively).

However, serum
proinflammatory cytokine levels had no prognostic role for outcome in PA patients.



Conclusions: Serum
levels of
proinflammatory cytokines were diagnostic markers in PA patients but not prognostic.
