PERİASETABULAR EWİNG SARKOMU OLGU SUNUMU: CERRAHİ TEKNİK VE LUMIC ASETABULAR İMPLANT İLE REKONSTRÜKSİYON

Abstract

Pelvik tümörlerin rezeksiyonu sonrası meydana gelen periasetabular defektlerin rekonstrüksiyonu ortopedik onkolojik cerrahide oldukça zorlu bir durumdur. Geleneksel rekonstrüksiyon yöntemleri mekanik ve fonksiyonel açıdan yetersiz kalabilmektedir. LUMIC® (Implantcast, Buxtehude, Germany) asetabular implant pelvik rezeksiyon sonrası rekonstrüksiyonda iyi sonuçların elde edildiği bir implant olarak kullanılmaktadır. Bu olgu sunumunda, 28 yaşında periasetabular Ewing sarkomu sebebiyle tip-II pelvik rezeksiyon ve Lumic asetabular implant ile rekonstrüksiyon uygulanmış bir kadın hastanın klinik ve fonksiyonel sonuçları sunulmaya çalışıldı. Bu olgunun diğer bir özelliği ise tip 2 asetabuler rezeksiyonun trokanterik osteotomi yapılmadan yapılabilmiş olamsıydı. Bu sebeplede erken postop dönemde ve takiplerinde (2. yıl) aktif kalça abduksiyonu korunabildi ve Musculoskeletal Tumor Society fonksiyonel değerlendirme skalasına göre iyi kalça fonksiyonu elde edilebildi. Sonuç olarak LUMIC® acetabular implant ile memnun edici klinik ve radyolojik sonuçlar elde etmek mümkündür. Tip 2 asetabuler rezeksiyon vakadan vakaya değişebilmekle birlikte trokanterik osteotomy yapmadan da yapılabilir olduğu gösterilmiştir.

PERIACETABULAR EWING’S SARCOMA A CASE REPORT: SURGICAL TECHNIQUE AND RECONSTRUCTİON WİTH LUMIC ACETABULAR IMPLANT

Abstract

Reconstruction of
periacetabular defects following pelvic tumor resection is a demanding
procedure in orthopaedic surgical oncology. Conventional reconstruction
procedures may have dissatisfying mechanical and functional results. More
recently
developed,
Lumic acetabular implant (Implantcast,
Buxtehude, Germany)
has been used for
acetabular reconstruction after type 2 pelvic resection.
 We aimed to present a 28-year-old female
patient who underwent a reconstruction surgery with a Lumic acetabular implant
following a type-II pelvic resection due to periacetabular Ewing’s sarcoma.
Another important feature of the patient was the surgery is performed without
trochanteric osteotomy. Therefore early postoperative period and in the second
postoperative year, the patient was able to active hip abduction, with good
fonctional score according to Musculoskeletal Tumor Society Rating Scale.
As a conclutions, satisfactory functional and radiological results can be
obtained with Lumic
® acetabular implant.
On the other hand we show that type 2 resection can be achived without
trochanteric osteotomy.
