D Vitamini Replasmanı ile İnsülin İhtiyacı Azalan Tip 2 Diabetes Mellitus Olgusu

Abstract
Son
yıllarda yapılan epidemiyolojik çalışmalarda D vitamini eksikliği ile diabetes
mellitus gelişimi arasında ilişki gösterilmiştir. Deneysel çalışmalar da D
vitaminin diabetes mellitus patogenezinde rolü olabileceği görüşünü desteklemektedir.
Ayrıca D vitamini replasmanının uzun dönemde diabetes mellitus gelişme riskini
azaltabileceği bildirilmektedir. Vitamin
D’nin insülin direnci ve beta hücre disfonksiyonu üzerine reseptörü üzerinden
direkt ve kalsiyum homeostazını etkileyerek indirekt etkileri olabileceği
belirtilmektedir. Bu olgu
sunumumuzda, bazal-bolus insülin tedavisi alan ve kan şekeri regülasyonu
sağlanmakta zorlanan 49 yaşında tip 2 diyabetes mellituslu bir kadın hastanın D
vitamini replasmanı ile insülin ihtiyacının azaldığını ve kan şekeri
regülasyonun daha kolay sağlandığını bildirdik.
