Abstract

Objective: Evaluation of long-term results of primary trabeculectomy operationMethods: The cases that are followed up with diagnosis of glaucoma in Okmeydanı Training and Research Hospital Eye Clinic between January 2000 and December 2001 were evaluated retrospectively. All of the cases, despite maximum therapy, have high intraocular pressure (IOP), undergone primary trabeculectomy operation, are followed at least 6 months and regularly followed through 10 years were included in this study. IOP with or without medical treatment 18mmg or less than 18 mmHg accepted as successful. IOL pressure measured with applanation tonometry. Results: 89 eyes of 70 cases were included in this study. The cases included in the study, 42 male (60%), and 28 (40%) were female. The mean age was 63.65±12.18 years. Preoperative intraocular pressure determined as 30.36 ± 3.2 mmHg. In the follow up examination mean intra ocular pressure was 15.31 ± 1.2 mmHg at 1st month, 15.47± 1.1mmHg at 3rd month, 15.02±1.8 mmHg at 6thmonth, 15.34± 2.1 mmHg at 1st year, 15.82 ± 2.1mmHg at 2nd year, 17.06 ± 2.3mmHg at 5th year and 18.02 ± 2.2 mmHg at 10th year. Statistical analysis of these data showed significant decreased of intra ocular pressure in the post operative period in compare to the preoperative period, 1st month, 3rd month, 6th month, 1st year, 2nd year, 5th year an 10th year (p < 0.01). The follow-up period in the study was 91.10 ± 40.15 months (6-120 months).Conclusion: Primary trabeculectomy can be considered as an alternative treatment procedure especially in patients who does not use drugs regularly and unable to attend regular medical examination. Trabekülektomi cerrahisinin uzun dönem sonuçları

Abstract

Amaç: Primer trabekülektomi ameliyatının uzun dönem sonuçlarının değerlendirilmesi

Yöntemler: Okmeydanı Eğitim Araştırma Hastanesi Göz Kliniğinde Ocak 2000 – Aralık 2001 tarihleri arasında glokom tanısı ile izlenen, geçmişinde maksimal tedaviye rağmen yüksek göz içi basıncı (GİB) nedeniyle primer trabekülektomi öyküsü olan olgular retrospektif olarak incelendi. Postoperatif dönemde en az 6 ay boyunca takip edilmiş olup, 10 yıl boyunca düzenli takiplere gelen hastalarda çalışmaya dahil edildi. GİB ilaçlı veya ilaçsız 18 mmHg ve altı olgular başarılı olarak kabul edildi. GİB aplanasyon tonometrisi ile ölçüldü.

Bulgular: Bu çalışmaya 70 olgunun 89 gözü dahil edildi. Çalışmaya dahil edilen olguları 42’si erkek (%60), 28’i kadındı (%40). Hastaların ortalama yaşı63,65±12,18 yıl idi. Preoperatif ortalama göz içi basıncı 30,36 ± 3,2 mmHg olarak ölçüldü. Olguların kontrol muayenelerinde ortalama GİB değeri 1.ayda 15,31 ± 1,2 mmHg, 3.Ay 15,47± 1,1mmHg, 6,ay 15,02±1,8mmHg,1. yıl 15,34± 2,1 mmHg, 2.yıl 15,82 ± 2,1 mmHg, 5.yıl 17,06 ± 2,3 mmHg ve 10. yıl 18,02 ± 2,2 mmHg olarak ölçüldü. Bu verilerin istatistiki analizinde postoperatif 1.ay, 3.ay, 6.ay, 1.yıl, 2.yıl, 5.yıl ve 10.yıldapreoperatif döneme göre göz içi basıncında anlamlı bir düşüş olduğu tespit edildi. (p< 0.01) Çalışmaya alınan tüm olguların takip süresi 91,10 ± 40,15 aydı (6 ay – 120 ay).

Sonuç: Primer trabekülektomi özellikle uyumsuz ilaç kullanımı olan hastalarda ve düzenli tıbbi muayeneye gelemeyecek hastalarda alternatif olarak düşünülmesi gereken bir tedavi prosedürüdür.
