Different Physiological Nutrition in a Patient with Proximal Ostomy Without Resection

Parenteral nutrition is the
most commonly used treatment for short bowel syndrome and oral nutrition should
be encouraged in appropriate patients. A 47-year-old male patient underwent
emergency surgery for small bowel perforation due to lymphoma. Proximal and
distal small bowel loops were brought as an rifle barrel in abdominal ostomy.
Oral feeding solution containing medium chain fatty acids was started on the
first postoperative day. Small bowel
contents to the ostomy bag were given to the distal bowel loops with a
catheter. The patient did not need postoperative parenteral nutrition. Six
weeks later, the patient's ostomy was closed and discharged without any
problem. Intestinal integrity is achieved by transporting the contents of the
small intestine from the proximal bowel loops to the distal segments. Thus, the
patient was protected from malnutrition and its complications

Kısa barsak sendromlu hastaların beslenmesi
için en sık kullanılan tedavi yöntemi parenteral nütrisyon olup, uygun
hastalarda oral beslenme teşvik edilmelidir. Erkek, 47 yaşındaki hasta
lenfomaya bağlı incebarsak perforasyonu nedeniyle acil olarak ameliyat edildi.
Proksimal ve distal incebarsak ansları tüfek namlusu şeklinde batına ostomi
yapılarak ağızlaştırıldı. Postoperatif 1.gün orta zincirli yağ asitleri içeren
oral beslenme solüsyonu başlandı. Ostomi torbasına gelen incebarsak içeriği bir
kateter yardımı ile distal barsak ansına verildi. Hastanın postoperatif
parenteral beslenme ihtiyacı olmadı. Altı hafta sonrasında hastanın ostomisi
kapatıldı ve sorunsuz taburcu edildi. Proksimal barsak anslarından gelen incebarsak
içeriğinin distal segmentlere ulaştırılması ile barsak bütünlüğü sağlanmış olup
hasta malnütrisyon ve komplikasyonlarından korunmuştur.
