Treatment of Grade 3 Halluks Rigidus Cases: Distal Metatarsal Dorsiflexion Osteotomy Combined with Cheilectomy Short-Term Period Radiological and Clinical Outcomes

Abstract

Özet
Giriş: Çalışmanın amacı, evre 3 halluks rigidus olgularında eklemi korumak için kullanılan bir teknik olan distal metatarsal dorsifleksiyon osteotomisi yönteminin kısa dönem radyolojik ve klinik sonuçlarını analiz etmektir.
Gereç ve Yöntemler: Retrospektif çalışma Ocak 2018-Ocak 2023 tarihleri arasında Adana Şehir Eğitim ve Araştırma Hastanesi'nde gerçekleştirildi. Çalışmaya en az altı aylık konservatif tedavinin başarısız olduğu ve çeliektomi ile kombine edilmiş distal metatars osteotomi ameliyatı ile tedavi edilen halluks rigidus evre 3 yetişkin olgular dahil edildi. Yaş, cinsiyet, ameliyat öncesi Coughlin ve Shurnas derecelendirme skorları, Amerikan Ortopedik Ayak ve Ayak Bileği Derneği (AOFAS) Halluks skoru ve Ayak ve Ayak Bileği Yetenek Ölçümü (FAAM) skorları kullanılarak yapılan fonksiyonel klinik değerlendirmeler, ameliyat yöntemi ve komplikasyonlar not edildi.
Sonuç: Çalışma grubunun (n=12) ortalama yaşı 54.2±8.7 idi ve yaşları 44 ile 68 arasında değişmekteydi. Ayak başparmağı hareket açısının son değerlendirmeleri, dorsifleksiyon, plantar fleksiyon ve toplam hareket değerlerinin ameliyat öncesi ölçümlere göre anlamlı olarak arttığını, ancak altıncı ve on ikinci aylar arasında her üç açının da azaldığını gösterdi (p<0.001). Tüm olgular arasında, ortalama AOFAS Halluks ve FAAM Günlük skorları anlamlı olarak iyileşmiştir (p<0.001). Birinci MTP eklemin radyolojik değerlendirme sonuçları, nihai ortalama genişliğin ameliyat öncesi ölçüme kıyasla anlamlı derecede arttığını, ancak altıncı aya kıyasla değerin azaldığını gösterdi (p<0.001).
Tartışma: Evre 3 halluks rijidusun cerrahi tedavisinde, çeliektomi ile kombine edilmiş distal metatars osteotomi, birinci MTP'nin hareket açıklığını artırmakta ve klinik ve radyolojik iyileşme sağlamaktadır. Ancak, 3'ten farklı derecelendirilmiş halluks rijidus hastalarında yapılan çalışmaların sonuçlarına dayanarak distal metatarsal dorsifleksiyon osteotomisi planlamadan önce dikkatli bir değerlendirme yapılması gerekir. Ayrıca, intraoperatif %50 veya daha fazla sağlam kıkırdak oranı bulgusu cerrah için motive edici bir gösterge olarak düşünülmelidir.
