SÜNNET OPERASYONU YAPILAN ÇOCUKLARIN VE EBEVEYNLERİNİN KAYGI DÜZEYLERİNİ ETKİLEYEN FAKTÖRLER

Abstract

AMAÇ: Sünnet için uygulanan cerrahi prosedürler hızlı ve basit olmasına rağmen, araştırmalar sünnetin ebeveynlerde ve çocuklarda korku, kaygı ve rahatsızlığa neden olduğunu göstermektedir. Çalışmamız ile hastanemizde tıbbi olmayan nedenler ile sünnet operasyonu yapılan çocukların ve ebeveynlerinin operasyon öncesi dönemde kaygı seviyelerini ve yüksek kaygıya etki eden faktörleri belirlemeyi amaçladık.
GEREÇ VE YÖNTEM: Çalışmamıza Afyonkarahisar Sağlık Bilimleri Üniversitesi Hastanesi Çocuk Cerrahi ve Üroloji Anabilim Dalı’na başvuran, 1 Nisan 2023 - 1 Mayıs 2023 tarihleri arasında elektif sünnet uygulanan 0-18 yaş arası 100 olgu ve ebeveyni alındı. Çalışmaya dahil edilen çocukların kaygı düzeyleri; çocukların ameliyat öncesi kaygılarını ölçmek amacıyla geliştirilmiş modifiye Yale Preoperatif Anksiyete Skalası (mYPAS) ile değerlendirildi. Ebeveynlerin kaygı düzeyini değerlendirmek için Durumluk-Süreklilik Kaygı Ölçeği (DSKÖ) kullanıldı.
BULGULAR: Olguların yaş ortalaması 21,4 ± 22,8 aydı. Olguların %85’inin mYPAS skoru 30’dan daha yüksek iken %15’ininki düşük olarak bulundu. Çocuğun yaşı, hastaneye yatış ve geçirilmiş operasyon öyküsünün bulunup bulunmadığı, ebeveynlerin yaşı, eğitim düzeyi ve ekonomik durumu gibi özelliklerin çocuğun operasyon öncesi kaygı düzeyini değiştirmediği görüldü. Ebeveynlerin durumluk ve sürekli kaygı düzeyi ile çocuğun kaygı düzeyi arasında pozitif yönde anlamlı bir korelasyon saptandı (sırasıyla; r=0,331 p=0,001, r=0,266 p=0,008).
SONUÇ: Çalışmamızda kaygı düzeyi yüksek ebeveynlerin çocuklarında, sünnet operasyonu öncesi kaygı düzeyinin daha yüksek olduğu saptandı. Ebeveynlerin ameliyat öncesi kaygı düzeylerini azaltacak müdahaleler ve psikolojik destek, çocuğun kaygısını da düşürerek operasyon sürecine uyumu artırabilir

FACTORS AFFECTING THE ANXIETY LEVELS OF CHILDREN AND PARENTS UNDERGOING CIRCUMCISION SURGERY

Abstract

OBJECTIVE: Although surgical procedures for circumcision are quick and simple, research shows that circumcision causes fear, anxiety, and discomfort in parents and children. In our study, we aimed to determine the anxiety levels of children and their parents who underwent circumcision surgery for non-medical reasons in our hospital and the factors affecting high anxiety in the preoperative period.
MATERIAL AND METHODS: Our study included 100 cases aged 0-18 years, who applied to Afyonkarahisar Health Sciences University Hospital, Department of Pediatric Surgery and Urology and underwent elective circumcision between April 1-May 1 2023. Anxiety levels of the children included in the study; were evaluated with the modified Yale Preoperative Anxiety Scale (mYPAS), which was developed to measure preoperative anxiety of children. The State-Trait Anxiety Inventory (STAI) was used to assess the anxiety level of the parents.
RESULTS: The mean age of the cases was 21.4 ± 22.8 months. While the mYPAS score of 85% of the cases was higher than 30, it was found to be low in 15%. It was observed that the characteristics such as the age of the child, hospitalization and previous operation history, the age of the parents, education level and economic status did not change the preoperative anxiety level of the child. A positive and significant correlation was found between the state and trait anxiety levels of the parents and the anxiety level of the child (r=0.331 p=0.001, r=0.266 p=0.008, respectively).
CONCLUSIONS: In our study, it was found that the anxiety level before the circumcision operation was higher in the children of parents with high anxiety levels. Interventions and psychological support that will reduce parents' anxiety levels before surgery can reduce the child's anxiety and increase compliance with the operation process.
