Abstract

Purpose: In this study, the effect of copper, zinc, copper/zinc and ceruloplasmin ratios on prognosis is examined on a patient group with colorectal cancer. Method: This study is planned as a prospective, cross-sectional case control study. At the general surgery policlinics of ministry of health Ankara education and research hospital between the dates July 2006-April 2007, 35 patients are involved in the study who are identified with colorectal cancer diagnostics. These patients are between the ages of 25-75 (with average of 57), 22 male and 13 female. As the control group, 20 (11 male, 9 female) healthy people without any clinical complaints and laboratory symptoms between the ages 25-64 (with average of 49) are selected among hospital personnel. Copper and zinc levels in blood are observed with samples taken at preoperative phase for the patient group, and at a random time for the control group. Results: Tumor localizations for male patients were 29% at rectum, 34% at other parts of the colon; and for women patients were 13% at rectum and 14% at other parts of the colon. A significant relation is detected between tumor phase and serum copper (p=0.001) and zinc (p=0.001) levels. A significant relation is detected between advanced tumor phase and serum ceruloplasmin and serum copper/zinc ratios (p=0.001). A distinctive decrease in serum zinc level is observed in patient group. Conclusion: The contribution of our study to the literature is, we show that in addition to the parameters like CEA15.5, CEA 19.9, Ca 50, ferritine that are used for tumor markers, the elements like copper, zinc can be used not only as a single parameter, but also separately as copper, zinc, ceruloplasmin and Cu/Zn ratios for evaluation. We think that using these ratios together for evaluation provides more valuable data for early diagnostic, determination of effectiveness for the treatment and determination of prognosis

Kolorektal Kanserlerde Serum Bakır, Çinko ve Seruloplazmin Oranlarının Prognostik Değerleri

Abstract

Amaç: Bu çalışmada, kolorektal kanserli hasta grubunda; bakır, çinko, bakır/çinko ve seruloplazmin oranlarının prognoza olan etkisi araştırılmaktadır. Yöntem: Bu çalışma prospektif, kesitsel bir vaka kontrol çalışması olarak planlandı. Temmuz 2006-Nisan 2007 tarihleri arasında S.B. Ankara Eğitim ve Araştırma Hastanesi Genel Cerrahi polikliniklerinde kolorektal kanser tanısı almış 25-75 yaş arası (ortalama 57), 22’si erkek ve 13’ü kadın olmak üzere toplam 35 hasta çalışmaya dahil edildi. Kontrol grubu olarak hastane personelinden 25-64 yaş arası (ortalama 49) herhangi bir klinik şikayeti ve laboratuar bulgusu olmayan, sağlıklı 20 (11 erkek, 9 kadın) kişi seçildi. Kan örnekleri hasta grubunda preoperatif dönemde, kontrol grubunda herhangi bir zamanda alınarak serum seruloplazmin, bakır ve çinko seviyelerine bakıldı. Bulgular: Tümör lokalizasyonları erkek hastaların %29’unda rektumda, %34’üde kolonun diğer bölgelerinde, kadın hastaların ise %13’ünde rektumda, %14’ünde kolonun değişik bölgelerindeydi. Dukes sınıflamasına göre yapılan, tümör evresi ile serum bakır (p=0.001) ve çinko (p=0.001) seviyeleri arasında anlamlı bir ilişki saptandı. İleri tümör evresi ile serum seruloplazmin ve serum bakır/çinko oranı arasında anlamlı bir ilişki olduğu saptandı (p=0.001). Hasta grubunda serum çinko düzeyinde belirgin azalma olduğu tespit edildi (p=0.001). Sonuçlar: Bizim çalışmamızın litaratüre olan katkısı ise tümor markırı olarak kullanılan CEA15.5, Ca 50, ferritin gibi birçok parametreye ilaveten bakır, çinko gibi elementlerden yalnız bir parametre bağlı değil; çinko, bakır, seruloplazmin ve Cu/Zn oranının birlikte değerlendirmeye alınması özellikle kolerektal kanserlerde tarama, erken tanı, verilen tedavinin etkinliğini saptama ve prognoz belirlemede daha değerli bilgiler verdiği kanatindeyiz
