Multipl Skleroz Tanılı Bireylerde Subjektif Yorgunluk Değerlendirme Yöntemleri: Derleme

Abstract

Multipl Skleroz’a özgü yorgunluk fiziksel aktivite ile artan ve dinlenme ile azalan bir karakter gösterebildiği gibi, dinlenme ve uyumakla dahi geçmeyen bir karakter kazanarak patolojik yorgunluk haline gelebilir. Hastalığa özgü yorgunluk algısını etkileyen faktörlerin anlaşılabilmesi için daha detaylı sınıflandırmalar yapılmaktadır. Fizyolojik ve patolojik yorgunluk olarak sınıflandırılabileceği gibi kaynağına (santral veya periferal yorgunluk), süresine (akut ve kronik yorgunluk), lokalizasyonuna (genel veya lokal yorgunluk) ve ortaya çıkış nedenine (primer veya sekonder yorgunluk) göre çeşitli başlıklar altında da incelenebilir. Multipl Skleroz’a özgü yorgunluğun yaygın olarak kabul edilen bir tanımının bulunmaması ve birçok boyutunun olması değerlendirilmesini zorlaştırmaktadır. Bu nedenle hastalarda araştırılmak istenen faktöre ve pratik uygulanabilirliğine göre bir veya birden fazla ölçek kullanılması çalışmalarda ya da klinikte yorgunluğu çok boyutlu olarak değerlendirmek için tercih edilebilir. Bu derleme ile yorgunluğu değerlendirmek için geliştirilen ölçeklerin Türkçe adaptasyon, geçerlik ve güvenirlik çalışmaları yapılıp yapılmadığı, ölçeklerin soru sayısı ve alt boyutları, özellikleri ve ek bilgilerini araştırılarak detaylı olarak incelendi.

SUBJECTIVE FATIGUE ASSESSMENT METHODS IN INDIVIDUALS WITH MULTIPLE SCLEROSIS: REVIEW

Abstract

Multiple Sclerosis-specific fatigue may show a character that increases with physical activity and decreases with rest, or it may become pathological fatigue by acquiring a character that does not pass even with rest and sleep. In order to understand the factors affecting the perception of fatigue specific to the disease, more detailed classifications are made. It can be classified as physiological and pathological fatigue, and can also be examined under various headings according to its source (central or peripheral fatigue), duration (acute or chronic fatigue), localisation (general or local fatigue) and cause of occurrence (primary or secondary fatigue). The fact that there is no widely accepted definition of fatigue specific to Multiple Sclerosis and that it has many dimensions makes it difficult to evaluate. For this reason, the use of one or more scales according to the factor to be investigated in patients and practical applicability may be preferred to evaluate fatigue multidimensionally in studies or in the clinic. In this review, the Turkish adaptation, validity and reliability studies of the scales developed to evaluate fatigue, the number of questions and sub-dimensions of the scales, their features and additional information were examined in detail.
