Tip 2 Diyabetli Bireylerde Fizyolojik Harcama İndeksi, Fonksiyonel Kapasite ve Klinik Belirteçler

Abstract
Tip 2 diyabeti olan kişilerde mekanik ve metabolik yetersizliklere bağlı olarak enerji harcaması etkilenmektedir. Bu çalışmanın amacı fonksiyonel egzersiz kapasitesinin değerlendirmesi sırasında enerji harcamasını ve tip 2 diyabeti parametrelerinin ilişkisinin değerlendirilmesidir. Çalışmaya 17 tip 2 diyabetli hasta dahil edildi. Laboratuvar değerleri ve antropometrik ölçümleri kaydedildi. Fonksiyonel kapasitesi altı dakika yürüme testi ile değerlendirildi. Enerji harcaması fizyolojik harcama indeksi ile altı dakika yürüme işi kullanılarak değerlendirildi. Fizyolojik harcama indeksi altı dakika yürüme testi ve istirahatteki kalp hızı farkının hıza bölünmesiyle hesaplandı. Altı dakika yürüme işi altı dakika yürüme mesafesinin vücut ağırlığının çarpılmasıyla hesaplandı. Ortalama fizyolojik harcama indeksi 0,37 ±0,18 (atım/m)’idi. Fizyolojik harcama indeksi boy, vücut kütle indeksi, açlık kan glukozu, total kolesterol ile ilişkiliydi (p<0,05). 6DYT işi yaş, boy, c-reaktif protein, bel kalça oranı, yağ yüzdesi, yağsız vücut kütlesi ile ilişkiliydi (p<0,05). Tip 2 diyabetli hastalarda fonksiyonel kapasite sırasındaki enerji harcaması klinik parametrelerle ilişkilidir. Enerji harcamasının alternatif yöntemleri olan fizyolojik harcama indeksi ve altı dakika yürüme işinin farklı klinik belirteçlerle ilişkili olması açısından birlikte incelenmesi önerilmektedir.

Abstract
Energy expenditure is affected due to mechanical and metabolic deficiencies in individuals with type 2 diabetes. The aim of this study was to evaluate the relationship between energy expenditure during the assessment of functional exercise capacity and type 2 diabetes parameters. Seventeen patients with type 2 diabetes were included in the study. Laboratory and anthropometric values were recorded. Functional capacity was assessed using a six minute walk test. Energy expenditure was assessed using the physiological cost index and six minutes walk test work. Physiological cost index was calculated by difference between resting heart rate and heart rate at the end of the six minute walk test (beats), divided by average velocity (m/min). The six minutes walk work was calculated using six minute walk distance (m) x body weight (kg). The mean physiological cost index was 0.37 ± 0.18 (beats/m). Physiological cost index was associated with height, body mass index, fasting blood glucose and total cholesterol (p <0.05). Six minutes walking work was related to age, height, c-reactive protein, waist-hip ratio, fat percentage, lean body mass (p <0.05). Energy expenditure during functional capacity in patients with type 2 diabetes is associated with clinical parameters. It is recommended to examine the both of physiological cost index and the six-minute walking test which is an alternative method of energy expenditure because of theirs relation to different clinical markers.
