Acute mediastinitis is a rare condition with a high mortality rate. One
of the causes of acute mediastinitis is perforation due to esophageal
foreign bodies. Acute mediastinitis can easily develop after the
perforation because of vasculare and physiological structure of the mediastinum and it is a very high mortality condition. In case
of acute mediastinitis due to esophageal foreign bodies, medical and
surgical treatment should be started as soon as possible. In this
article, we present a case in which a patient registered late (on 18th day) and was diagnosed with acute acute mediastinitis and empyema due to fish bone stick into the esophagus and died within minutes after beggining of the treatment.
Akut mediastinit nadir görülen, mortalite oranı
oldukça yüksek bir durumdur.Akut mediastinit sebeplerinden birisi de
özafagus yabancı cisimlerine bağlı perforasyondur. Perforasyon sonrası
mediastenin vasküler ve fizyolojik yapısı nedeni ile akut mediastinit kolaylıkla
gelişebilir ve mortalitesi yüksektir.
Özofagus yabancı cisimlerine bağlı akut mediastinit olgularında zaman
geçirmeden medikal ve cerrahi tedaviye başlanmalıdır. Yazımızda özofagusa balık
kılçığı saplanması sonrası geç dönemde (18. gün) başvuran, akut mediastinit, ampiyem tanısı koyarak
tedaviye başladığımız ve dakikalar içerisinde ex olan olgumuzu sunulmaktadır.
