Psychosocial evaluation and quality of life of patients with vitiligo.

Abstract

Objective: The aim of this study was to assess the psychogenic factors such as social anxiety, self-esteem, anxiety and depression and sexuality in vitiligo patients and investigating the effect of disease on quality of life.
Method: Fourty clinically diagnosed cases with vitiligo admitted to Departments of Dermatology Outpatient Clinic of Training and Research Hospital were compared with fourty healthy control subjects both sexes were 16-65 years. The subjects were assessed with The Hospital Anxiety and Depression Scale (HADS), Arizona Sexual Experiences Scale (ASEX), Liebowitz Social Anxiety Scale (LSAS), Skindex-29 and Rosenberg Self Esteem Scale (RSES).
Results The percentage of psychiatric treatment in vitiligo patients was significantly higher than control group. The levels of social anxiety, anxiety and depression scores were significantly higher in vitiligo group than the control group.Vitiligo group had low self-esteem. Among females and males, sexual drive and orgasm of the vitiligo group was more difficulty and total score of sexual experiences was significantly higher than the control group. İn vitiligo group quality of life was statistically significantly lower than control group according to higher levels of social anxiety, patients with lower self-esteem, higher levels of anxiety and depression scores and smokers.
Conclusion: The results of our study support the previous findings suggesting that the rate of psychiatric morbidity is higher and life quality is lower in patients with vitiligo than healthy controls.

Vitiligo Hastalarının Psikojenik Faktörler ve Yaşam Kalitesi Açısından Değerlendirilmesi

Abstract

AMAÇ: Bu çalışmanın amacı vitiligo hastalarında sosyal kaygı, benlik saygısı, anksiyete ve depresyon ve cinsellik gibi psikojenik faktörlerleri değerlendirmek ve hastalığın yaşam kalitesi üzerine etkisini araştırmaktır.
YÖNTEM: 16-65 yaşları arasında 40 vitiligo hastası ve 40 sağlıklı kişi çalışmaya dahil edilerek, Hastane anksiyete ve depresyon ölçeği (HADÖ), Arizona cinsel yaşantılar ölçeği formu (ACYÖ), Liebowitz sosyal kaygı ölçeği (LSKÖ), Cilt ve saç bozukluğu olan hastalarda performans ve yaşam kalitesi ölçeği (Skindex-29) , Rosenberg benlik saygısı ölçeği (RBSÖ) ile kesitsel olarak değerlendirilmiştir.
BULGULAR: Vitiligo grubunda psikiyatrik tedavi alanların oranı kontrol grubundan istatistiksel olarak anlamlı derecede yüksek saptanmıştır. Vitiligolu hastaların sosyal kaygı düzeyleri, anksiyete ve depresyon puanları kontrol grubuna göre anlamlı düzeyde yüksek saptanmıştır. Vitiligo grubunda benlik saygısı düşük saptanmıştır. Vitiligosu olan kadınlar cinsel açıdan kontrol grubuna göre anlamlı derecede daha zor uyarılıp, daha zor orgazm olmaktaydı ve cinsel yaşantılar toplam puanı kontrol grubuna göre anlamlı derecede daha fazlaydı. Vitiligolu erkekler cinsel açıdan kontrol grubuna göre anlamlı derecede daha az istekli, daha zor uyarılmaktadırlar. Vitiligolu erkeklerin cinsel yaşantılar toplam puanı kontrol grubuna göre anlamlı derecede daha fazladır. Vitiligo grubunda sosyal kaygı düzeyleri daha yüksek olan hastalarda, benlik saygısı daha düşük olan hastalarda, anksiyete ve depresyon puanları daha yüksek olan hastalarda ve sigara kullananlarda yaşam kalitesi istatistiksel olarak anlamlı ölçüde daha düşük saptanmıştır.
SONUÇ: Çalışmamızın sonuçları vitiligo hastalarında psikiyatrik morbiditenin daha yüksek ve yaşam kalitesinin daha düşük olduğu yönündeki önceki bulguları desteklemektedir.
