Mental Foramenin Panoramik Radyografi ve Konik Işınlı Bilgisayarlı Tomografi Görüntüleri Üzerinde Morfolojik Analizi

Abstract
Amaç: Bu çalışmanın amacı; bir grup Türk alt popülasyonunda mandibular dişlerin köklerine göre mental foramenin (MF) horizontal, vertikal konumunu ve anterior loop (AL) görülme sıklığını panoramik radyografi ve konik ışınlı bilgisayarlı tomografi (KIBT) görüntüleri üzerinden değerlendirmekti. Ayrıca her iki görüntüleme yöntemi arasında AL görülme sıklığını tespit etmede fark olup olmadığını belirlemekti.
Gereç ve Yöntemler: Bu çalışmaya dahil edilme/dışlama kriterini sağlayan 16 ile 80 yaşları arasında 435 hasta dahil edilmiştir. Mental foramenin horizontal konumu mandibular 1. premolar diş (PM1), mandibular 2. premolar diş (PM2) ve mandibular 1. molar dişe göre, vertikal konumu da PM1’nin ve PM2’nin apekslerini birleştiren yatay çizgiye göre belirlenmiştir. AL görülme sıklığı ise inferior alveolar sinirin üç farklı yapıda sonlanmasına göre değerlendirilmiştir. Verilerin normal dağılıma uygunluğu Kruskal Wallis ve Shapiro Wilk testi ile analiz edilmiş olup, kategorik değişkenler arasındaki farklılıkların saptanmasında ki-kare analizi kullanılmıştır. Gerekli koşullarda Yates düzeltmesi ve Fisher Exact testi kullanılmıştır. p<0.05 istatistiksel olarak anlamlı kabul edilmiştir.
Bulgular: Çalışmada MF’in en sık karşılaşılan horizontal konumu PM1’nin uzun ekseni ve PM2’nin uzun ekseni arasında bulunmuştur. MF’nin en sık karşılaşılan vertikal konumu PM1’in ve PM2’nin apekslerini birleştiren yatay çizginin altında izlenmiştir. İnferior alveolar sinir MF’de sonlanma şekli en yaygın olarak doğrusal şekilde bulunmuştur. Panoramik radyografi görüntüleri ile değerlendirildiğinde % 17.3 oranında, KIBT görüntüleri ile değerlendirildiğinde ise % 21.4 oranında AL bulunmuştur.
Sonuç: İntraforaminal bölgeye yapılan cerrahi prosedürlerden önce MF’nin konumu ve AL varlığının tespit edilmesi bu bölgedeki olası komplikasyonları önlemek için önemlidir. MF bölgesine yapılacak olan cerrahi prosedürlerde KIBT’nin AL varlığı açısından diş hekimlerine daha iyi rehberlik edebileceği düşünülmüştür.

Abstract
Background: The aim of this study was to evaluate the horizontal and vertical position of the mental foramen (MF) relative to the roots of the mandibular teeth and the incidence of anterior loop (AL) in a group of Turkish subpopulations on panoramic radiography and cone beam computed tomography (CBCT) images. It was also to determine whether there was a difference in detecting the incidence of AL between both imaging methods.
Methods: This study included 435 patients aged 16 to 80 years who met the inclusion/exclusion criteria. The horizontal position of the mental foramen was determined according to the mandibular 1st premolar tooth (PM1), mandibular 2nd premolar tooth (PM2) and mandibular 1st molar tooth, and its vertical position was determined according to the horizontal line connecting the apex of PM1 and PM2. The incidence of AL was evaluated according to the termination of the inferior alveolar nerve in three different structures. The suitability of the data to normal distribution was tested with the Kruskal Wallis and Shapiro Wilk test, and Chi-square analysis was used to determine the differences between categorical variables. Yates correction and Fisher Exact test were used when necessary. p<0.05 was considered statistically significant.
Results: In the study, the most common horizontal position of MF was found between the long axis of PM1 and the long axis of PM2. The most common vertical location of MF was traced below the horizontal line connecting the apex of PM1 and PM2. The termination of the inferior alveolar nerve in MF was most commonly found to be linear. The rate of AL was found to be 17.3% when evaluated with panoramic radiography images, and 21.4% when evaluated with CBCT images.
Conclusion: Determining the location of MF and the presence of AL before surgical procedures in the intraforaminal region is important to prevent possible complications in this region. It is thought that CBCT can better guide dentists in terms of the presence of AL in surgical procedures to be performed on the MF region.
