Hospital Management and Leadership in Caotic Conditions: A Qualitative Research on the Covid-19 Epidemic

Abstract

The Covid-19 pandemic affecting the whole world and industries has brought new facts and has forced the managers/leaders to respond to the crisis in a new way by changing their usual styles. The aim of the study is that in hospitals, which are one of the most important elements of the health system in the Covid-19 pandemic, it is to examine the areas that managers have to manage and the difficulties they confront during the epidemic management, what kinds of choices and decisions they have to make in these areas and how they lead the process. For this purpose, semi-structured interviews are held with ten senior executives of public hospitals in Turkey and the decoded data are analyzed through the content analysis. According to the analysis results, during the epidemic management in hospitals; leadership characteristics of managers, planning, continuity of basic health services, human resource management, communication and plans for the next epidemic come to the fore. In addition, the structure of the crisis is defined, and the advantageous and disadvantageous situations encountered by the hospitals during the fight against the epidemic are defined. Besides the analysis of the management and leadership qualities in hospitals during the Covid-19 outbreak, it is thought that the study may serve as a guide for preparation for future outbreaks.

Kaotik Durumlarda Hastane Yönetimi ve Liderlik: Kovid-19 Salgını Üzerine Nitel Bir Araştırma

Abstract

Tüm dünyayı ve sektörleri etkisi alan Kovid-19 pandemisi yeni olguları beraberinde getirmiş ve yönetici / liderleri tarzlarını değiştirerek krize yeni bir şekilde yanıt vermeye zorlamıştır. Bu çalışmanın amacı; Kovid-19 pandemisinde sağlık sisteminin en önemli unsurlarından olan hastanelerde, yöneticilerin salgın yönetimi sırasında yönetmek zorunda kaldığı alanların ve karşılaştıkları zorlukların neler olduğu ve bu alanlarda ne tür tercihler yapmak ve ne tür kararlar almak durumunda kaldıklarını ve sürece nasıl liderlik ettiklerini incelemektir. Bu amaç doğrultusunda Türkiye’de hizmet vermekte olan 10 kamu hastanesinin üst düzey yöneticisi ile yarı yapılandırılmış mülakat görüşmeleri gerçekleştirilmiş ve bu görüşmelerin yazıya dökülmüş belgeleri üzerinden içerik analizi yöntemi kullanılarak analizler gerçekleştirilmiştir. Analiz sonuçlarına göre hastanelerde salgın yönetimi sürecinde; yöneticilerin liderlik özellikleri, planlama, temel sağlık hizmetlerinin devamlılığı, insan kaynağı yönetimi, iletişim ve bir sonraki salgına hazırlık planları boyutları ön plana çıkmıştır. Ek olarak krizin yapısı tanımlanmış ve hastanelerin türüne göre salgınla mücadele sırasında karşılaştıkları avantajlı ve dezavantajlı durumlar tanımlanmıştır. Bu çalışmanın Kovid-19 salgınıyla mücadele sırasında hastanelerdeki yönetim ve liderlik boyutlarını analiz etmenin yanında ileride yaşanabilecek salgınlar için yol gösterici nitelikte olacağı düşünülmektedir.
