Palyatif Bakım ve Ölümle Yüzleşme

Öz

Tıptaki ilerlemeler sonucu yaşam beklenti süreleri uzamaktadır buna bağlı olarak artık

daha ileri yaşta ve daha komplike ölümlerle karşılaşılır olmuştur. Hastalar ve yakınları

özellikle terminal dönemdeki vakalarda çoğu zaman ölümü kabullenir olmuşlardır

ancak daha konforlu ölüm beklentileri yükselmektedir. Sağlık çalışanları ise hastaların

son dönemlerine ait kararlarını uygulamak istemekle birlikte mevcut yasal düzenlemeler

bazı isteklerin uygulanmasına uygun değildir. Özellikle yurt dışında DNR ‘Do Not

Resuscitate’ veya CPR (Kardiyo Pulmoner Resussitasyon) negatif hasta kavramları yaygınlaşmaktadır.

Fakat mevcut yasal düzenlemelerle kardiyopulmoner arrest gelişen hastaya

müdahale etmemek, hastaya pasif ötenazi uygulamakla eşdeğer tutulmakta ve cezası

1-3 yıl arası hapis olabilmektedir. Palyatif bakımda hastaya biyolojik destek sağlanırken,

kimi hastalar da manevi olarak ölüme hazırlanmaktadır. Mevcut tedavilerin artık

yaşam süresini uzatmayacağını öğrenen hasta, palyatif bakım ünitelerinde kendi çizdiği

sınırlar içerisinde çevresi ile vedalaşmaktadır. Yaşam hikayemizin başlangıcı ve seyrinden

de önemlisi bitiş şeklidir. Palyatif bakım ünitelerinde geriye dönüşsüz hastalara

güzel ve onurlu bir ölüm imkânı hazırlanmalıdır.
