Türk Çocuklarında İki Yöntem ile Diş Yaşı Tayininin Birbiri ile Karşılaştırılması

Öz

Amaç: Diş yaşı (DY) tayini yöntemlerinin geçerliliği farklı popülasyonlarda test edilmektedir. Bu çalışmanın amacı İstanbul’da yaşayan Türk çocukların DY tayininde Demirjian (D) ve Haavikko (H) yöntemlerinin karşılaştırmalı olarak incelenmesidir.
Yöntem: Etik onayı alındıktan sonra, çalışma kapsamında İstanbul Üniversitesi Diş Hekimliği Fakültesi Oral Diagnoz ve Radyoloji Anabilim Dalı arşivindeki 8-14 yaş grubu çocuklara ait panoramik radyografiler dahil edilme kriterlerine göre seçilerek retrospektif olarak incelenmiştir. Bireylerin kronolojik yaşları (KY), panoramik röntgenin alındığı tarihten doğum tarihlerinin çıkarılması ile hesaplanmıştır. Her bir çocuğun DY’si D ve H yöntemlerinin her ikisi ile de belirlenmiştir. DY ve KY arasındaki farkın ve mutlak farkın %95 düzeyinde güven aralıkları verilmiştir. Doğrusal regresyon modelleri her bir cinsiyet ve her bir DY yöntemi için ayrı ayrı oluşturulmuştur. İstatistiksel testler için anlamlılık seviyesi p<0,05 olarak belirlenmiştir.
Bulgular: Dahil edilen 293 çocuğun KY ortalaması 10,98±1,74 yıldır. Olguların %51,2’si (n=150) erkek, %48,8’i (n=143) kız çocuklardan oluşmaktadır. Her iki cinsiyet grubu için de H yöntemi ile elde edilen sonuçların ortalaması, D yöntemi ile elde edilen sonuçların ortalamasına göre KY’ye daha yakın olarak çıkmıştır. Buna göre en iyi tahmin sırası ile erkeklerde H yöntemi (-0,39 yıl), erkek ve kızlarda H yöntemi (-0,46 yıl), kızlarda H yöntemi (-0,52 yıl) ile bulunmuştur (p<0,05). Regresyon formülünde sadece erkekler çocuklar için H yönteminin (R2=0,84); sadece kız çocuklar için ise D yönteminin daha başarılı olduğu (R2=0,77) bulunmuştur.
Sonuç: 8-14 yaş grubundaki tüm çocuklar için H ve D yönteminin diş yaşını tayin etmede tek başlarına kullanılmalarının doğru sonucu vermediği, kız ve erkek çocuklar için ayrı olmak üzere yaş dilimlerine göre farklı yöntemlerin kullanılmasının gerektiği görülmüştür.

A Comparison of Two Different Dental Age Estimation Methods in Turkish Children

Öz

Objective: Validity of dental age (DA) estimation methods are tested in different populations. The aim of this study is to compare the Demirjian (D) and Haavikko (H) methods in Turkish children.
Methods: After obtaining ethical approval, panoramic radiographs of 8-14 year-old children in the archives of Department of Oral Diagnosis and Radiology were selected analyzed retrospectively. Chronological ages (CA) of the individuals were calculated by substracting the birth dates from the date of the panoramic x-ray was taken.DA of each child was determined by both D and H methods.Confidence intervals of 95% of the difference between DA and CA and the absolute difference are given.Regression models are formulated separately for each gender and DA method.The significance level was set to be p<0.05.
Results: The mean CA of the 293 children is 10.098±1.74 years. 51.2% (n=150) of the cases were boys and 48.8% (n=143) were girls. The mean value of the results obtained with the H method for both gender was close to CA compared to the mean results obtained with the D method. The best estimate was found with H method (-0.39 years) in boys, H method (-0.46 years) in boys and girls, H method (-0.52 years) in girls (p<0.05).H method (R2=0.84) was found to be more successful for boys and D method method was found to be more successful (R2=0.77) for girls.
Conclusion: It was observed that the use of only H or D method for age estimation for all children in 8-14 age group does not give accurate results. There is a need to investigate different methods.
