Penetrating head injuries can originate many intentional or unintentional events. We presented a case of six year-old girl developed complication of menengitis after intranasal penetration of a wooden pencil. Emergency physician should be aware the possibility of residual foreign bodies in the management of patients diagnosed with intracranial injury, hemorrhage and infections because these serious complications may originate from transnasal penetrating foreign bodies. The advanced imaging modalites and early consultations of ear-nose-throat specialist and neurosurgery are necessary in these cases.
