In this case report, the restoration of a segmental mandibular defect which occurred after the resection of a recurrent squamous cell carcinoma lesion with implant-supported hybrid prosthesis after reconstructed with free fibular flap was described. A 48-year-old male patient with partial glossectomy and mandibular reconstruction was referred to clinic due to maxillary and mandibular partial-edentulism. Clinical and radiological examinations revealed teeth with decreased bone support because of chronic-periodontitis and maxillary impacted canine. After extraction of impacted canine and other teeth, a wax-up try-in was performed to determine final teeth position and vertical dimension. Due to significant vertical tissue loss, hybrid-prostheses were planned. Four implants were inserted to each jaw. After osseointegration, open-tray impressions were taken, metal-frameworks were fabricated, and their passive fit were checked clinically. Centric relation, vertical dimension, phonetic and aesthetic of tooth setting were verified intraorally. After occlusal adjustments, the final prostheses were delivered to patient and screw holes were covered. The patient was satisfied with stability and esthetics of his prostheses and did not have any problems regarding prostheses during 6 months of service. The standard treatment approach for oral cancers is the surgical resection of pathological tissues with a certain sum of peripheral healthy tissue and reconstruction of hard and soft tissue defects secondary to the excision using different grafts. However, due to inadequate tissue support after reconstruction, conventional prosthetic restorations cannot have sufficient retention and function. Therefore, implant-supported hybrid prostheses represent good alternative for edentulous jaws with significant hard and soft tissue loss.
